Eligibility

We believe all hospice-eligible patients and those who love them deserve the end of life. Hospice eligibility is defined as a life expectancy of six months or less if the illness runs its normal course.

Who is eligible?

Hospice eligibility is determined by prognosis rather than diagnosis. Hospice serves any patient who is clinically eligible for the benefit, including but not limited to people living with: Alzheimer's disease, heart disease, COPD, stroke, Parkinson's disease, ALS, and cancer.

If you would like to get in touch with us, we're here for you 24 hours a day, 7 days a week, and 365 days a year to answer any questions you or your loved ones may have. Use the form below, begin a chat at the bottom of the screen, or call 1.844.464.0411 to connect.
