Diagnosis & Prognosis

Many people think hospice is just for those with Cancer. The reality is Hospice serves any patient who is clinically eligible for the benefit, including, but not limited to, people living with Alzheimer's disease, heart disease, COPD, stroke, Parkinson's disease, ALS and cancer. Eligibility is defined as a life expectancy of six months or less if the illness runs its normal course.

Who should get hospice?

Signs That May Indicate A Need For Hospice Services

If the patient is:
 Not "bouncing" back
 Losing weight or gaining weight uninitentionally
 Showing a decrease in appetite
 Having frequent infections
 Encountering shortness of breath
 Experiencing unplanned hospitalizations or ER visits
 Finding it harder to perform daily activities (ADLs)

How We Can Help You

The mission at Heart of Hospice is to serve all hospice-eligible patients in the way they desire to be served. We have an entire team ready to provide care and comfort to you. We will work together to develop a plan of care that honors your goals upon admission. However, your individual plan of care is not set in stone. As your needs change, we will collaborate with you and your physician to give you the services that best fit your situation at that time. We will not ask you to give up hope, comforting treatments or your doctor.

How To Receive

Call us at (844) 464-0411. Anyone can refer a patient to our service. One of our team members will visit you to assess your needs and work with your doctor to determine if you qualify for the benefit. Meeting with our team is provided without charge, and you will learn about the benefits which you may be entitled to receive so that you can make an informed decision about your care.
